Patient-Centered Medical Homes

Given healthcare reform and the continued shortage of primary care physicians and other providers, it is critical to rethink how to deliver care efficiently. One care model that has gained recent interest is the patient-centered medical home (PCMH) model. The medical home model can reduce emergency department use and hospital admissions through improved care coordination, patient engagement, and application of evidence-based care protocols, while providing additional revenue opportunities.

We have assisted medical groups, hospital-based practices, faculty practice plans, and public entities in evaluating their readiness for PCMH and developing innovative practice models that meet the needs of the patient population. Our experts have extensive experience with practice redesign and workflow optimization, incorporating the use of technology to improve efficiency, and meeting the requirements of the PCMH model and Meaningful Use. Our process involves a readiness gap analysis as well as creating an action plan to meet National Committee for Quality Assurance (NCQA) recognition requirements (e.g., facilitation of the decision-making process regarding structure and workflow design) for those who want to become a NCQA certified medical home. We can also provide on-site management for the NCQA application process as well as implementation support.
